If you’re a caretaker or a family member who’s interested in scheduling an appointment for your loved one with special needs, everything you’ll need to know is compiled on this page. Below, we’ve included some basic information about forms and what to expect during the first visit.
Every patient’s visit can vary based on the reason why they’re visiting us, but for all of our patients, we treat them in a comfortable wheelchair (either their own, or one of ours) that will be tilted backwards with our Versatilt Wheelchair Lift. We also provide dental sedation for all of our patients as necessary, so they can feel comfortable and at-ease during their appointment. We welcome patients to bring in any comfort items that will help them feel calmer, too!
We ask that all of our patients fill out our forms either online or download them, fill them out at home, and bring them in. If you have any difficulty or need help, please reach out to our team.

If you have any problem getting these files to print properly, please call our dental office and we will gladly mail or fax them to you. As a courtesy we will file with your dental insurance on your behalf. You should be prepared to pay any co-payments. In addition, payment of sedation services is required at the time of the appointment.
It is not uncommon that our patients receive assistance from a third-party agency, and we often coordinate our treatments with this in mind.
